| There
are an increasing number of forms, letters and notes being
required by patients for a variety of reasons. Please appreciate
that these forms take a considerable amount of time to complete
and can only be completed by Dr. Kelpin outside the normal
office hours. At any given time Dr. Kelpin may receive a large
number of forms all at once and as such it is difficult to
plan ahead and budget time to complete these forms.
Most forms
will require you to be seen in the office in order that details
may be obtained from you, physical examinations may be done
and appropriate follow up can be scheduled.
Please
advise the office staff if you have any forms that require
completion at the time that you make an appointment and present
the form to the staff when you arrive for your visit. This
will help us to schedule the day and to be able to obtain
all the necessary information to complete the form at your
office visit.
There
are fees for the completion of all forms, notes etc. These
fees can be seen by going to the Fee Guide for Private Services
section of this website.
Most
forms seem to have a time frame in which they need to be completed
and all forms will be completed in the order in which they
are presented to us at the office regardless of the circumstances
involved. It is seldom possible to complete any forms on the
same day as you are seen to complete them so please plan ahead
in order to help us avoid disappointing you in that circumstance.
In order that we may be able to complete your form for you
in as timely as manner as possible please try to adhere to
the following:
- Clarify
with the company, school, insurance agency, etc. as to what
form or information or physical examination is required
in order to meet their needs.
- Call
us as soon as you have this information, or receive the
form, to make us aware of what needs to be done.
- Make
an appointment to come in as soon as possible to discuss
the form and obtain any needed evaluation from Dr. Kelpin,
so that he may complete the form for you.
- Please
pay for the form as soon as possible, as forms will not
be sent out of our office until fully paid for.
